How to Create a Submission Form

Introduction to Submissions Forms

The Submission Form in AcceleratorApp allows you to collect important data or submissions from learners in your courses. This customizable tool helps streamline data collection for assessments, assignments, and feedback, and can be used to gather key insights from your community.

Feature Location

To create a Submission Form, navigate to the Side Bar Menu > Forms Management, click Create New Form, name your form, and select Submission as the form type. If needed, add additional languages from the dropdown menu.

Step-by-Step Actions

 1. Adding Questions

Use the Drag and Drop functionality to add various types of questions:

  • Text fields (Long and One Liner) – For open-ended submissions.
  • Multiple choice – For pre-defined response categories.
  • Rating scales – To evaluate the quality of a submission.
  • Yes/No questions (Single Choice) – For binary responses.
  • Attachment questions – To allow users to submit files.
  • Number questions – For numerical data submissions.
  • Date questions – To collect date-based data.
  • User Add Row questions – Allow users to submit multiple entries.

2. Formatting Elements

Improve the form layout by adding:

  • Section Headers – To organize the form into logical parts.
  • Subheaders – To introduce specific areas of the form.
  • Paragraphs – To provide instructions or extra context.

3. Setting Validations

Ensure accurate submissions by setting validations:

  • Character limits for text responses.
  • Required fields for essential questions.
  • Email format validation for contact info.
  • Numerical range for number-based fields.
  • Date restrictions for date-related fields.

5. Applying Conditional Rules

  • Create personalized forms based on responses:
  • Trigger additional fields based on previous answers.
  • Show specific questions for learners or mentors depending on their selection.

6. Converting Fields into Custom Fields

6. Sharing and downloading

You can share the application form as a preview link that allows external users to visualize the form without needing an account. However, this link is for preview purposes only and does not allow submissions.

If needed, you can download a PDF version of the form to share it offline.
